Temporary List Closure

We are currently unable to accept new patients apart from babies born to those already registered here.

This is due to exceptional circumstances and we will return to accepting new patients as soon as possible. The practice is focused on delivering safe high quality medical care and we are deeply grateful to all the staff who are working to ensure.

New Patient Registration

We welcome new patients. If you would like to register with us please complete the registration form, new patient questionnaire and Summary Care Record Consent form which can all be found below. Once these have been completed please deliver the forms to the surgery. Forms are also available at the surgery reception desk. Please bring two forms of identification with you to complete your registration with us. One needs to be photographic ID and the second needs to be dated within the last three months and show your current address within our practice area (see map opposite).

We like to see newly registered patients for a health check and to introduce ourselves to you. Please book for a New Patient Appointment with reception.
