To use the online prescription service you must first request access codes and instructions from the surgery so that you can register. You can collect these in person or telephone us and we will post them to you. Once you have these you can click on the green ‘Register, Create an Account’ box and create your account and password.
If you forget your password or User ID you can re-set or be reminded of them by clicking on the ‘I forgot my User ID or Password’ links on the www.patient.co.uk sign–in page. We cannot reset your password for you.
Alternatively you can hand in or post your prescription counterfoil to reception with the items you need clearly marked. This is for your safety as it will reduce the chance of errors in prescriptions issued. We do accept fax requests, but would prefer you to use the counterfoil as above where possible.
Prescriptions will be available for collection two working days after the request has been made. If you are housebound and require a prescription to be delivered please let us know when you order the prescription.
North Yate Pharmacy shares our buidling and you can be accessed through reception. Local chemists Boots (Yate Shopping Centre and West Walk Surgery) and Lloyds (Yate Shopping Centre, Courtside and Chipping Sodbury Surgeries) also collect prescriptions daily from the surgery so that once you have ordered a prescription the items can be collected direct from the chemist. Please allow two working days from your request before collection. Please let us know where you would like your prescription sent when you order it.
