
New Patient Registration
We welcome new patients. If you would like to register with us please complete the registration form, new patient questionnaire and Summary Care Record Consent form which can all be found below. Once these have been completed please deliver the forms to the surgery. Forms are also available at the surgery reception desk.
There are practical reasons why we might need to be assured that people are who they say they are, or to check where they live. Seeing some form of ID will help to ensure the correct matching of a patient to the NHS central patient registry, thereby ensuring any previous medical notes are passed onto us. Therefore please bring two forms of identification with you. One should be photographic ID and the second a utility bill dated within the last three months that shows your current address within our practice area (see map opposite).
